Surgical and Endoscopy Booking Resources
Maintaining a Surgical Registry requires standardization among hospitals and surgical facilities in how information about surgical cases is collected and reported. To that end, the Ministry provides a single surgical booking form for use by surgeons, which ensures that the same information is collected province-wide.
In 2025, a provincial booking form for endoscopy procedures also became available.

Surgical Booking Form
There is a single surgical booking form for the whole province, but Regina and Saskatoon booking packages include the provincial form plus customized physician order sections.

Diagnosis-based Prioritization
Saskatchewan uses a diagnosis-based prioritization method to assign priority for procedures in the surgical booking system, based on standard categories of patient diagnosis. The Saskatchewan Provincial Division of Surgery, Provincial Department of Obstetrics and Gynecology, Saskatchewan Health Authority, and the Ministry of Health support this initiative.
Surgeons must provide the following data when submitting a surgical booking form: a six-character diagnosis code and the associated diagnosis description, which you will find on the Saskatchewan Surgical Diagnosis Code List.
Pediatric diagnosis codes (based on PCATS) are also available.

Orthopedic surgery diagnosis-code-builder
While most surgical specialties are provided with a list of complete diagnosis codes, orthopedic surgeons individually select the patient’s diagnosis, the affected part of the body, and the level of functional disability for each surgical case. In total there are more than 2,000 possible combinations of diagnosis, body part and level of urgency. To streamline this process for orthopedic surgeons, a code-builder function was developed by data experts at the Ministry of Health and is available here. This is a web-based tool and no download is required.

2. Endoscopy Booking Resources
Endoscopy Booking Form
There is a one-page provincial endoscopy booking form for the province, along with a version that includes a physician order section on a second page. Regina and Saskatoon endoscopy booking packages also include the provincial form plus customized respiratory and/or physician order sections.

3. Surgical Registry Policies and Procedures
Booking offices in the province’s 23 surgical facilities follow standard processes for managing bookings for surgery and endoscopy procedures, as set out in the Surgical Registry Regulations, 2024.
The Surgical Registry Policies and Procedures Manual, 2025 outlines standards for the staff of booking offices related to processing bookings, scheduling procedures and completing case documentation in the surgical information system. It also clarifies roles and expectations for operational leaders, surgeons and surgical patients to ensure fair and efficient use of surgical resources to meet everyone’s needs.
