Educator Separation Form

School divisions are encouraged to complete an Educator Separation Form to identify educators who are no longer employed or will have a break in service.

Reasons for a break in service include:

Death Professional Misconduct
Deferred Salary Leave Resigned
Educational Leave Superannuated
Illness/Disability Suspended by School Division
Leave of Absence with Pay Teacher Exchange
Leave of Absence without Pay Temporary Contract Expired
Parenting Leave Terminated by School Division
