Employers can complete the Application for Reimbursement of Professional Development Expenses Form for all staff who attended an event.
Those who attended the event must provide:
- Required receipts
- Proof of registration
- A copy of the Certificate of Participation (if available)
More than one application form can be submitted, but the total of all applications cannot exceed your facility's maximum Professional Development Grant.
Applications must be submitted within two months following each event attended.
